Whether you’re [self-employed](https://feather-insurance.com/en-ww/blog/international-health-insurance-freelancer), [moving abroad with family](https://feather-insurance.com/en-ww/blog/international-health-insurance-families), [working as a digital nomad](https://feather-insurance.com/en-ww/blog/international-health-insurance-digital-nomad), or [looking to retire](https://feather-insurance.com/en-ww/blog/international-health-insurance-senior) somewhere warm, there are plenty of great reasons to move abroad. But wherever you’re moving, there’s a good chance you’ll need a visa. If so, you’ll probably have to think about health insurance sooner rather than later.

Valid health insurance is a prerequisite for most visa applications. This means you’re going to have to think carefully about what policy you require before making your application.

## United States: health insurance requirements for visas

For expats in the US, there are two main types of visas:

* **J-1:** The main visa used by expats coming to the US for study and work-related activities. It covers activities like employment, internships, or research projects. Generally, applicants must be sponsored by an organization to qualify.   
* **F-1:** This is the main visa used by [international students](https://feather-insurance.com/en-ww/blog/international-health-insurance-student). If you’re pursuing a full-time academic course at a United States college/university, there’s a good chance this is the visa you’ll need.

These visas have different health insurance requirements. Let's look at each one separately.

### United States: J-1 visa health insurance requirements

Most expats in the United States will enter on a J-1 visa. To do so, you will need to demonstrate private health insurance that meets [the following requirements](https://isso.ucsf.edu/j-visa-health-insurance-requirements-and-options): 

* Medical benefits of at least $100,000  
* Maximum deductibles of $500 per accident or illness  
* Maximum co-payments of 25%  
* Minimum medical evacuation coverage of $50,000  
* Minimum emergency repatriation coverage of $25,000  
* Coverage for pre-existing conditions (after a reasonable waiting period)  
* Includes coverage or benefits for activities related to the reason for your visa

Travel insurance policies are rarely accepted here. The chosen insurance provider must also be accredited by one of the four following credit agencies: 

* AM Best (Minimum A-)  
* Insurance Solvency International (Minimum A-i)  
* Standard & Poor’s (Minimum A-)  
* Weiss Ratings (Minimum B+)

### United States: F-1 visa health insurance requirements

Unlike J-1, the F-1 visa does not have health insurance requirements at the federal level. 

Nonetheless, the vast majority of students on this visa will need health insurance anyway. The only difference is it will be managed and enforced by your college/university. 

By default, most colleges/universities require you to have international health insurance. In general, you will be auto-enrolled into a Student Health Insurance Plan (SHIP) that is managed by your education provider. By default, these must be valid for the entire period of study. 

You may opt out in some cases, but only if you can prove equivalent levels of coverage through an alternative health insurance policy.

## Schengen area: health insurance requirements for visas

The situation in the Schengen area can be complex because there are several visa types to consider. At the same time, the rules aren’t exactly the same across all Schengen countries. 

For the purposes of this article, we’ll focus on two main types of visa: 

* **Type C Schengen visa:** Known as the ‘Schengen’ visa or ‘short-stay’ visa. This is the main type of visa used by tourists. It is valid for a maximum of 90 days within a 180-day period. The health insurance requirements for this visa are fairly minimal. You will generally require travel insurance for the Schengen visa.   
* **Type D Schengen visa:** This is the long-stay visa, which applies to anybody staying between 90 days and a year. Unlike the short-stay visa, it is designed for use primarily within a specific country, rather than the entire Schengen area. This visa is most relevant to anybody looking to move to a specific Schengen country as an expat. As such, the health insurance requirements differ notably between different countries. 

As well as sharing these visas, there are several other principles that Schengen countries have in common: 

* **Public and private healthcare:** Most European countries operate a mixed public and private insurance healthcare system. If you’re moving to a European country, you may become eligible for public healthcare while you’re here. If so, you may not need separate, long-term medical insurance. However, public healthcare entitlements don’t become valid immediately. Either way, you’ll still need valid insurance at the point of entry.  
* **Schengen baseline:** A baseline set of health insurance requirements is defined at the EU level. For instance, rules such as ‘€30,000 minimum coverage’ are consistent across the Schengen area.  
* **Type C vs. Type D visa requirements:** Type C requirements do not differ widely across the Schengen area. Type D requirements are generally higher and differ noticeably between countries.

### Schengen visa travel insurance requirements (Type C)

The health insurance requirements for a Type C Schengen visa are defined in [the EU Regulation (EC) No 810/2009, Article 15](https://eur-lex.europa.eu/eli/reg/2009/810/oj/eng). Here are some of the main requirements:

* The policy must offer minimum coverage of €30,000 and be valid across all Schengen countries  
* It should also be valid for the entire period of the person’s intended stay/transit  
* You should be covered for repatriation expenses. This can relate to urgent medical attention, emergency hospital treatment, or death

These rules are less strict than those for Type D visas, because they’re only designed for short stays. Travel insurance is generally valid here – though repatriation coverage must be included.

### Long-stay visa requirements in Germany

Germany has demanding health insurance requirements for long-stay (Type D) visa applicants. Visa applicants must demonstrate valid health insurance at the point of entry. You must also arrange a long-term health insurance solution, which may differ from the policy you used to enter. There are essentially two options for what that long-term arrangement can be:

* **State social security:** This is known as *‘gesetzliche Krankenversicherung’* (GKV). This is the default option for anybody employed by a German organization who earns less than €77,400 per year – including expats. If you’re an expat with a confirmed job offer, your health insurance will only need to cover you until your GKV entitlements become valid.   
* **Private insurance:** Also known as *‘private Krankenversicherung’* (PKV). This is required for anybody who does not qualify for GKV. If you’re an expat, you will likely require private insurance unless you’re employed by a German organization. If you are employed and earn over €77,400 a year, you may choose a PKV insurance package and opt out of the GKV. 

Regardless of the long-term arrangement, you’ll need to demonstrate some form of valid health insurance to enter Germany. If needed, a PKV policy must offer equivalent coverage as Germany's GKV.

Often, expats who aren’t employed by a German organization will not have access to the GKV system. This is particularly relevant to retirees and other non-lucrative expats. If this applies to you, you must maintain full private health insurance indefinitely. 

For self-employed expats in Germany, the situation is a little different. Here, you can opt in to the GKV, but only if you’ve previously been registered under the GKV or another European public healthcare system. In this case, you can choose either GKV or PKV. If you haven’t been registered under a European public healthcare system, you’ll need a long-term PKV policy. 

Given the choice, many self-employed people still prefer PKV as it can result in lower payments in the short term.

### Long-stay visa requirements in France

In France, the long-stay (Type D) visa is known as VLS-TS (Visa de long séjour valant titre de séjour). This enables you to live in France for between four months and a year. Here are some of the [most important health insurance requirements](https://www.frenchentree.com/living-in-france/french-long-stay-visas-which-health-insurance-do-you-need/):

* Travel insurance policies are not accepted  
* The health insurance policy should cover the entire visa length, regardless of how long you want to stay in France  
* Medical expenses, hospitalization, and repatriation must all be covered  
* Minimum coverage of €30,000  
* The policy must not exclude pre-existing conditions  
* There must also be no co-pays or deductibles

The situation in France becomes more complex when we take into account the country’s public healthcare system. This is known as *‘Protection Universelle Maladie’* (PUMa). In France, you’ll become eligible for public healthcare after three months in the country. If you apply for and secure public healthcare, the above requirements are no longer valid. 

However, France’s public healthcare system only covers around 70% of standard medical costs. French residents and citizens therefore take out an additional ‘top-up’ insurance (known as *‘mutuelle’*). This covers the remaining costs in most cases.

If you qualify for public health insurance, you no longer require private insurance. In this case, you may choose to replace your international health insurance with top-up insurance instead. If you’re employed in France, you’ll generally be automatically enrolled in a mutual insurance policy.

### Long-stay visa requirements in Spain

Spain has similar requirements for long-stay visas to France. Here are some of the baseline details: 

* €30,000 minimum coverage  
* The insurer must be authorized to operate in Spain and issue a certificate in Spanish  
* No co-pays or deductibles  
* The insurance policy must cover all risks insured by Spain’s public healthcare system  
* The policy must be valid for at least a year and be maintained for the entire duration of the stay  
* Travel insurance is not accepted

For your visa to be accepted, you must have a valid health insurance policy that fulfills these requirements. 

Like with other European countries, you may qualify for public healthcare shortly after arriving. In Spain, you can do this by contributing to the local social security system – known as *‘Seguridad Social’*. 

If you’re employed by a Spanish organization, you will be enrolled into this by default. For freelancers, retirees, and other non-working expats, the situation is more complex. By default, you do not qualify for public healthcare. Therefore, the private health insurance requirements we listed above will apply. 

However, there is one caveat: *Convenio Especial*. This is a separate scheme that lets you pay into the social security system if you’re not already covered through employment. This is primarily designed for non-lucrative expats and freelancers who don’t pay into the system by default. You may apply for this after one year’s residence in the country. 

## United Kingdom: healthcare requirements for visa applicants

The United Kingdom has a different approach to healthcare and insurance than most European countries. 

Visa applicants do not require private health insurance. Instead, you are required to pay an up-front [Immigration Health Surcharge](https://www.gov.uk/healthcare-immigration-application) (IHS) when applying for the visa. This essentially buys access to the country’s public healthcare system – the [National Health Service](https://www.nhs.uk/) (NHS). 

However, the NHS does not cover all types of treatment. Opticians, dentists, hearing specialists, and prescriptions are not paid for by the NHS – even for UK citizens. Therefore, you may choose to take out private healthcare to cover these additional treatments.

The IHS currently costs a flat rate of [£1,035 a year](https://www.gov.uk/healthcare-immigration-application/how-much-pay#:~:text=%C2%A3776%20per%20year%20for%20students%2C%20their%20dependants%2C,for%20example%20%C2%A33%2C105%20for%20a%203%2Dyear%20visa.). This must be paid up-front, when you apply for your visa. Payment is a prerequisite for the visa being accepted. If the visa you’re applying for will last for multiple years, you must pay all in one go. 

Once you’ve paid this fee, you’ll have access to the full UK healthcare system from day one. You can then interact with it in the same way as British citizens.

So you should hopefully be in good company here. 

But when it comes to visas, healthcare, and other requirements, it’s easy to get overwhelmed by details. After all, there’s a good chance this is your first time living abroad. And the consequences of getting it wrong can be very daunting. The last thing you want is to find yourself with an unexpected bill or a rejected visa.

## What is international student health insurance? 

[International health insurance](https://feather-insurance.com/en-ww/blog/international-health-insurance-guide) is an umbrella term for any private health insurance used by expats. For students, it is often a requirement for your study visa – if you need one.

In reality, ‘international health insurance’ is a loose umbrella term that can mean different things. That’s because there are several factors that influence what health insurance you will require: 

* Which country you’re originally from  
* Where you’re studying  
* What visa (if any) is valid for your stay  
* What existing entitlements/insurance schemes you’re already covered under

If you’re studying in the US, for instance, you will almost certainly require some level of private health insurance. This is usually managed by your university/college. In the EU’s Schengen Area, you will likely need insurance at the point of arrival, but may become entitled to public healthcare during your stay. In the United Kingdom, most visa applicants pay an [Immigration Health Surcharge](https://www.gov.uk/healthcare-immigration-application) as part of the application. 

As you can see, international student health insurance can mean very different things to different people. 

## International health insurance vs travel insurance: What’s the difference?

International health insurance and travel insurance are not the same thing. That’s because the former is designed for expats and the latter is designed for short-stay visas. 

As a student, travel insurance is generally **not valid.** However, it may be accepted for short periods outside of your country of study – i.e. placements, projects, or vacations. 

## Why do students need international health insurance?

There are several different situations where students might need private international medical insurance: 

### Visa requirement 

The most common reason: Medical insurance is often a requirement of your study visa. These rules will differ depending on the country you’re studying in — see our [guide for visa applicants](https://feather-insurance.com/en-ww/blog/international-health-insurance-visa-seekers) for a broader walkthrough of the visa-side coverage rules. For instance, you may need a policy that's valid from before you apply for the visa, before you enter the country, or shortly after. It’s important to check these details beforehand, as getting it wrong can result in your visa being rejected. 

### Short-term coverage

In some countries, you are eligible for public social security coverage shortly after arriving. This is as little as three months in some cases. In this case, you will generally need a short-term international student health insurance plan. This should cover you between your arrival and your social security entitlements becoming valid. 

### Personal choice

You may also choose to take out medical health insurance to increase your coverage. This is particularly relevant if you have existing coverage through the country’s social security system. Otherwise, you might want greater coverage than what’s on offer through your college/university. 

### Short-term placement

If you’re traveling outside your country of study (e.g. vacation, project, or fieldwork), you might need to extend your medical coverage. This depends on the specific terms of your existing insurance policy. Some apply worldwide, and others may be limited to your country of study. 

### Dependents

If you’re bringing dependents with you during your study, you’ll likely need to insure them separately. If you’re entitled to a medical insurance policy through your college/university, they likely won’t be covered. ## What does health insurance for international students include? 

It’s impossible to create a definitive list of what international medical insurance covers and includes. That’s because the details will differ depending on several key factors. These include the type of policy you purchase, your country of origin, and where you’re staying. 

Nonetheless, certain treatments will be common in international student health insurance plans. Others are more common in higher-cost policies. Here are some of the main terms to look out for:

### Basic healthcare coverage

All international health insurance policies will include a baseline level of medical coverage. This could include medical expenses, doctor visits, surgeries, emergency care, and diagnostics. 

### Additional healthcare coverage

Often, additional healthcare requirements are only available in higher tiers of insurance. Depending on the location, this might include dentists, opticians, or prescriptions. 

### Repatriation

This covers the cost of your travel back to your country of origin in the case of death or medical emergency. This often isn’t included in basic packages – so it’s important to double-check this. In the Schengen Area, repatriation coverage is generally a visa requirement, so you may get caught off guard if your insurance doesn’t include it. 

### Preventative care

Your insurance may cover preventative treatments like routine screenings or vaccinations. This is more common in higher insurance tiers. 

### Global coverage

Different policies will apply in different areas. The most basic could just include your country of study. If you’re studying in Europe, you’ll likely have Schengen-wide coverage. Otherwise, worldwide coverage is more common in higher-priced policies. 

## What international health insurance is required for a visa application?

If you’re planning to study in another country, there’s a good chance you’ll require a visa to do so. And often, health insurance for international students is a requirement. 

The specific rules will depend on your country of study. However, it’s helpful to give a brief overview of the visa requirements in some key regions around the world:

### United States

In the United States, students are generally accepted under the [F-1 Student Visa](https://travel.state.gov/content/travel/en/us-visas/study/student-visa.html). You do not require private health insurance as a pre-requisite of this. 

Nonetheless, health insurance for college students is still required by most education providers. In this case, it is generally managed through your college/university. If so, you will be automatically enrolled in a student insurance plan. However, you can sometimes opt out if you can prove you have equivalent coverage elsewhere. 

### The Schengen Area

The situation in the European Union is more complex. This is because there are essentially two layers of rules for countries within the Schengen Area: 

* **Schengen:** A minimum set of visa/insurance requirements applied across the whole region.   
* **Individual countries:** EU countries also have their own visa/insurance requirements. By design, these must supersede the Schengen minimum. 

The baseline Schengen rules are set out in [Article 15 of the EU Visa Code](https://eur-lex.europa.eu/eli/reg/2009/810/oj/eng) (Regulation (EC) No 810/2009). This includes the following requirements:

* **Coverage**: Must have travel medical insurance covering emergencies and repatriation  
* **Scope:** All policies must include coverage for all Schengen countries  
* **Financial coverage:** At least €30,000

In reality, most countries will have stricter requirements than this. In Spain, for instance, you need to have private healthcare for your visa to be accepted. It must also be valid for the entire length of your course. This can mean students have to buy insurance upfront, before the visa is accepted. 

France has a similar but slightly different situation. Here, you must also have valid insurance before you enter the country. However, it usually only needs to be valid for a few months. This is because you are eligible for coverage under the country’s social security system after a short period.

However, the state system only covers a portion of the costs. Therefore, you’ll generally require additional top-up (or ‘mutuelle’) health insurance. 

### United Kingdom

In the United Kingdom, visa applicants do not need to have international health insurance or private health insurance of any kind. This is because private healthcare in the UK is comparatively limited.  

Instead, you have to pay an upfront surcharge, which effectively ‘buys’ access to the country’s public healthcare system. This is known as the Immigration Health Surcharge, [which costs £1,035 a year](https://www.gov.uk/healthcare-immigration-application/how-much-pay#:~:text=%C2%A3776%20per%20year%20for%20students%2C%20their%20dependants%2C,for%20example%20%C2%A33%2C105%20for%20a%203%2Dyear%20visa.) in 2026. 

The charge must be paid upfront for the entire length of the visa. If you’re planning to study a three-year degree, therefore, you’ll have to pay £3,105 for three years’ access.

## How do I choose the right international student health insurance?

> *“The gaps that catch people off guard are almost always the same.*

> *First, transition coverage. You’ve landed in your new country, but you’re not in the local system yet. In Spain, you need your NIE (Número de Identificación de Extranjero, your foreign ID number). In Germany, public insurance can take weeks to activate after starting a new job. People assume they’re covered from day one. They’re not.*
>
> *Second, pre-existing conditions when switching plans. You had coverage at home and you get a new plan abroad. But you then discover your chronic condition has a waiting period of 12–24 months before it’s covered again. A short gap between policies can reset the clock entirely.*
>
> *Third, repatriation and emergency evacuation. Most people don’t think about this until something goes wrong. By then it’s too late to fix it.”*

When you choose your international student health insurance policy, it’s important to research the options on the table. The last thing you want is to take out an insurance policy that’s not valid for your visa, and then get rejected anyway. At the same time, you probably don’t want to pay for insurance that you don’t need. 

So how do you work out what the right level of coverage is for you? Here are a few important tips: 

### Public healthcare

In several European countries, you will be eligible for public healthcare after you’ve spent some time in the country. However, this is not valid from day one. Check the requirements in your country of study. This will inform what type of insurance you need and how long it needs to last for. 

### Length of coverage

If you’re likely to be covered under a public healthcare system, you may only need short-term ‘transition’ coverage. Otherwise, some countries (like Spain) will require insurance to be valid for the entire length of your visa. Understanding what you need is crucial to making the right choice. 

### Repatriation

Many standard plans do not include repatriation. If you’re studying in the Schengen Area, these will not be valid for your visa. Elsewhere, it’s sensible to make sure this is included even if it’s not a requirement. That’s because emergency repatriation can be very expensive. The last thing you want is to be landed with a multi-thousand-dollar bill after a medical emergency. 

### Pre-existing conditions

Medical insurance packages often do not cover pre-existing conditions. If you have particular ongoing health conditions, it’s important to be aware of the implications here. In some cases, you may have to purchase more expensive insurance packages to ensure you’re covered. 

### Additional coverage

You may also wish to be covered for additional procedures like dentists, orthodontics, or hearing support. In these situations, you may need a more expensive international student health insurance plan. It’s important to thoroughly check the coverage implications of whatever plan you choose.

## What is international health insurance?

### How international health insurance works

When you take out a plan, you’re covered through a network of hospitals and clinics. Your insurer pays the provider directly or reimburses you after treatment. This depends on your policy, though.

International plans are designed to work outside your home country. Domestic health insurance isn't. Most international plans cover a defined region. For instance, worldwide excluding the USA, or worldwide including the USA. So, if you relocate within that region, your cover moves with you.

## International health insurance vs travel insurance

Here are the big differences: duration and scope. International health insurance is for long-term living abroad, travel insurance for short trips. Getting the distinction wrong can leave you under-insured.

Travel health insurance works for short trips where emergency-only coverage will do. But it doesn’t cover routine care or long-term conditions. Neither does it cover mental health support, maternity, or dental treatment beyond emergencies. In a nutshell: if you’re moving abroad or staying long-term, travel insurance won’t be enough.

### Transition coverage gaps

The transition period is riskier than people expect. You’re in a new place, adjusting to a new diet, new routines, new stress levels. 

Public healthcare isn’t always immediately accessible either. According to [OECD Health at a Glance 2023](https://www.oecd.org/en/publications/2023/11/health-at-a-glance-2023_e04f8239/full-report/waiting-times-for-elective-surgery_68501edc.html), in several countries more than 60% of people wait over a month just to see a specialist. And that’s for residents already registered in the system. New arrivals still working through the paperwork aren’t yet in the queue at all.

International health insurance starts from day one of your policy. There’s no waiting for local registration. Even in countries with public healthcare, the system can be slow and costly out of pocket. Many keep international cover even after they’re settled.

### Pre-existing conditions when switching plans

A pre-existing condition is any health issue you had before your policy started. Most insurers will exclude the condition or apply a waiting period. They may charge a higher premium, or decline to cover it entirely. It depends on your medical history.

Try not to leave a gap between your old policy ending and your new one starting. Even a few weeks without cover can mean starting a waiting period from scratch. Some insurers offer cover from day one, depending on your health history. Be honest on your health questionnaire. Leaving something out can void the whole policy.

### Repatriation and emergency evacuation limits

Medical evacuation flights can cost tens of thousands. Why? Factors like distance, medical complexity, and whether you need a medical escort.

When comparing plans, don’t just check that repatriation is ‘included’. Check the benefit limit. And does it cover a medical escort? Family members travelling with you? Repatriation of remains? Basic travel insurance rarely covers all of this adequately.

### Geographic coverage limits

Even ‘worldwide’ plans often have restrictions. US exclusions are common on mid-range plans. Most policies exclude conflict zones. Network limitations can mean treatment at non-approved hospitals is only partially reimbursed. It may not even be covered at all.

### Claim denials and documentation issues

Most international health insurance plans only pay out for treatment that’s “medically necessary”. In other words, a qualified doctor has determined it’s necessary to diagnose or treat a condition. That rules out elective or cosmetic procedures. Common reasons claims are denied beyond this:

* Lack of pre-authorisation: Many plans need insurer approval before elective procedures.

As a freelancer, there’s a good chance you’re used to complicated rules around tax, pensions, accounting, and much more. And wherever you’re living, healthcare is just another thing to add to this list. 

This is hard enough in your home country. But for expats, it’s even tougher. This is because there’s a whole range of questions you need to answer first. How do freelancers get health insurance? What policy do you need? What coverage should you take? How does this interact with your visa? 

In truth, the answer can be very different depending on where you’re moving to and from. Here, we explain how health insurance works for expat freelancers around the world. ## Why international health insurance matters for expat freelancers

In many countries, health insurance is a non-negotiable requirement – even for permanent residents. But as a self-employed expat, there are several reasons why it’s particularly important for you: 

* **Healthcare:** The first reason is the most obvious. Without health insurance, you risk needing emergency healthcare and not being able to afford it. At its worst, this can result in high out-of-pocket costs or being denied treatment.   
* **Visa requirements:** As an expat living abroad, you will likely require health insurance as a condition of your visa. In this case, an international health insurance policy can be the best way to meet these requirements. * **Employer contributions:** In some countries, employers are required to offer private healthcare insurance. As a freelancer, you unfortunately don’t qualify for this – a challenge shared by the unemployed and other groups without an employer plan. In practice, this makes it more likely that you’ll have to buy a long-term private policy that covers all your healthcare needs. 

Therefore, most self-employed expats will need some form of health insurance. But what does that include? 

## How do freelancers get health insurance: comparing the main options

For expats and [digital nomads](https://feather-insurance.com/en-ww/blog/international-health-insurance-digital-nomad), there are several different types of health insurance you need to consider. The same logic also applies to other groups going abroad, such as [students](https://feather-insurance.com/en-ww/blog/international-health-insurance-student): 

* **Travel insurance:** This is designed for vacations, work projects, or other self-contained trips. It focuses on emergency and acute treatment – and is only valid on a per-trip basis. Therefore, it isn’t suitable for expats or freelancers, unless you’re only staying short-term. In fact, many visas explicitly rule out travel insurance as an option.   
* **International insurance:** This is any insurance policy designed for expats. It aims to provide health coverage for those moving abroad either permanently or for more than a few months. Many expats will take out international health insurance policies to qualify for a visa. Then, they can transition to domestic public or private insurance after they’ve arrived.   
* **Domestic private insurance:** This is the standard private insurance used by citizens and long-term residents in a country. Long-term expats can use these policies alongside (or instead of) public healthcare policies.   
* **Domestic public healthcare:** Many countries also offer public healthcare coverage. This is particularly the case in the European Union (EU) and European Economic Area (EEA) countries. Generally, freelancers or expats will qualify for this after a certain period living in the country. In some cases, it is even a requirement.

## What international health insurance do you need around the world?

Getting the right health insurance policy for your situation isn’t straightforward. To get it right, there are several key questions you need to understand the answers to: 

* How do freelancers get health insurance?  
* What health insurance is required for your visa?   
* When do you need to secure the policy?  
* How long should it be valid for?  
* Can you transition to public healthcare coverage? And when?  
* Are public or private health insurance policies tax-deductible?

In truth, the answers to this will differ depending on the country you’re traveling to and where you’ve come from. Therefore, it’s helpful to discuss requirements in key locations around the world:

### United States

In the United States, there’s no single, dedicated visa for freelancers. Instead, one of these three options is most likely to apply: 

* **O-1A/O-1B**: For individuals with extraordinary ability in business, science, arts, or athletics. Allows self-employment with a US-based agent or sponsor.  
* **EB-1A/EB-2 NIW**: These are green card routes for those with extraordinary ability or national interest. It bypasses the need to be sponsored by an organization. These are permanent residency, not visas.  
* **E-2 Treaty Investor**: Technically for business investment, but used by some self-employed individuals. Very limited freelancing scope.

In practice, the second and third options on the list aren’t really designed for freelancers. This makes the O-1A/O-1B visa the most common route. 

The United States does not require private health insurance as a condition of these visas. Nonetheless, the cost of healthcare in the US means it is virtually essential to be covered. 

In the US, there is no universal public healthcare system. Therefore, private health insurance for expat freelancers is similar to that of local residents. The main difference is you won’t receive any employer-sponsored health insurance, which many in the US rely on. 

On the bright side, however, private insurance premiums are tax-deductible in the US. This applies to premiums for yourself, a spouse, and dependents. ### Germany

In Germany, freelancers are known as *‘Freiberufler’* or *‘Selbstständiger’*. There are two main visa routes for expat freelancers to enter the country: 

* **Freelancer Visa (*‘Freiberufler Visa’*):** The primary route for self-employed professionals (writers, artists, engineers, consultants, IT, etc.)  
* **Opportunity Card (*‘Chancenkarte’*):** A points-based visa for job/work-seeking, including exploratory self-employment. This isn’t a full freelancer visa, but it can be a stepping stone to one.

Germany has a two-part healthcare system. Most citizens and long-term residents are covered under the public healthcare system. This is known as GKV (*‘gesetzliche Krankenversicherung’*). Otherwise, employees who earn over €77,400 a year can opt for private insurance (*‘private Krankenversicherung’* or ‘PKV’) – though there is no minimum for freelancers. 

For expats, however, PKV remains the default setting. Germany requires all visa applicants to have private health insurance when they apply. The policy must offer the equivalent coverage as Germany’s public GKV system. 

The healthcare insurance also needs to be valid before you apply for the visa. Generally, this means you’ll need to purchase a policy before making the application. 

Most self-employed expats in Germany will need indefinite private health insurance. This is because most non-working expats do not qualify for public healthcare coverage. The main exception is if you’ve historically made social security contributions in an EU country. In this case, you may opt in to GKV, making your private insurance requirements redundant. 

In Germany, public healthcare (via GKV) is tax-deductible as ‘special expenses’ (*‘Sonderausgaben’*). Private contributions (via PKV) are only tax-deductible up to the equivalent GKV rate. 

### France

France has two main routes for expat freelancers looking to gain a visa: 

* **Long-Stay Visa (VLS-TS)** (*‘Visa de Long Séjour valant Titre de Séjour’*): This is the default visa for non-EU/EEA nationals planning to stay in France for between 90 days and a year.  
* **Talent Passport (*‘Passeport talent’*):** A four-year talent passport for entrepreneurs, investors, and independent professionals. It aims to award entry to those with a compelling project or established expertise.

Like most European countries, France requires you to have valid health insurance when you apply for your visa. The policy must offer the same level of coverage as the French public healthcare system. This is known as PUMA (*‘Protection Universelle Maladie’*).

In France, you can apply for public healthcare coverage after just three months’ residence in the country. If you register as self-employed in France, this becomes a legal requirement. When you qualify for PUMA coverage, any private health insurance you have is redundant and can be canceled. 

In practice, this means many freelancers only need short-term health insurance policies when they apply for the visa. However, even with public coverage, you will likely still require a top-up insurance policy (*‘mutuelle’*), as PUMA does not cover 100% of costs. 

However, French lawmakers are [currently reforming this law](https://www.relocate.world/en/articles/french-healthcare-fee-us-visitor-visa-2026) to ensure that non-EU nationals must contribute a ‘minimum healthcare contribution’ to qualify for PUMA coverage. At the time of writing, this law does not apply, but could be introduced as early as this year. 

Generally, social security and top-up insurance contributions are tax-deductible in France. 

### Spain

Spain also has two main visa routes for self-employed expats looking to relocate. Broadly, these depend on whether you expect most of your income to come from **inside** or **outside** of Spain: 

* **Self-employment visa (*‘Autónomo Visa’*):** This is the traditional route for non-EU nationals wishing to live and work in Spain as self-employed. Generally, this is better suited to those looking to register as a freelancer in Spain and stay long-term.   
* **Digital Nomad Visa (*‘Visado de nómada digital’*):** For remote workers and freelancers working primarily for clients outside Spain. This is a recent addition to the Spanish system and is now one of the most popular routes of entry for international freelancers. To be registered under this visa, [at least 80% of your income](https://www.exteriores.gob.es/Consulados/londres/en/ServiciosConsulares/Paginas/Consular/Digital-Nomad-Visa.aspx) must be derived from outside of Spain. 

In principle, the situation here is similar to in France. If you’re registering permanently, you’ll generally do so as an *Autónomo*. In this case, you must contribute to the country’s public healthcare system (*‘Seguridad Social’*). 

If you’re registered as a digital nomad, you’ll likely need full-time private insurance. It must cover your full visa period and be active when you apply. 

In Spain, public social security contributions are fully tax-deductible. However, private contributions are only partially deductible. By default, Autónomos can deduct up to **€500 a year** per insured person. In some cases, such as disabilities, this can be higher at **€1,500 a year**.

### United Kingdom

In the United Kingdom, the situation is different from other countries on this list. That is because public healthcare coverage is the default setting for most people in the country. This is done through the National Health Service or ‘NHS’.

Regardless of the visa you’re applying for, the situation is effectively the same. The majority of visa applicants will have to [buy access to the NHS up-front](https://www.gov.uk/healthcare-immigration-application/who-needs-pay) as a pre-condition of their visa being accepted. This is known as the ‘Immigration Health Surcharge’ (IHS). In 2026, the rate is £1,035 per year for most visa applications. If you’re applying for a multi-year visa, you’ll have to pay the IHS up-front for all years. 

If you’re staying in the UK for less than six months, you **do not** need to pay the IHS. Instead, you will simply be charged up-front for any health-related needs you have while you are here. You may choose to purchase private health insurance to cover this, but there is no requirement to do so. 

## How can freelancers choose the right international health insurance policy?

As a freelancer, getting the right international health insurance policy is not straightforward. To get it right, you need to thoroughly check the details. Then, take the time to understand their implications for your budget and healthcare coverage. 

Here are some tips to help you ask the right questions: 

### Check your visa requirements

In Europe, you’ll almost certainly require private healthcare for your visa to be accepted. Elsewhere, the situation may be more flexible. Either way, you should check the requirements before choosing the policy. 

### Check the cancellation terms

In some cases, you will qualify for public healthcare coverage shortly after arriving. Nonetheless, you will still need international healthcare insurance when you apply for the visa. In this case, the policy would become redundant shortly after. If this applies to you, you should ensure the policy has flexible cancellation policies so you don’t get stuck paying for something you don’t need. 

### What coverage do you need?

You should also consider what type of coverage you need in the short and long term. This could apply to opticians, dental/orthodontic care, or chronic conditions. In particular, international health insurance for pre-existing conditions can be expensive – this is especially relevant for [older freelancers and seniors](https://feather-insurance.com/en-ww/blog/international-health-insurance-senior). So it’s important to understand what these implications are before you start looking. 

Extra coverage generally comes with higher premiums. But it’s often worth it if you’re likely to need the treatment. Otherwise, you may prefer a lower up-front premium, though this risks higher out-of-pocket expenses in the long run. 

### Consider co-pays or deductibles

Co-pays or deductibles can help bring down your premium in the short term. But it can be a false economy. These additional costs can easily add up if you find yourself needing a particular type of treatment a lot. You should also be aware that co-pays and deductibles are not allowed for most visas in the Schengen area. If you’re considering going down this route, tread carefully and check the details thoroughly. 

### Decide whether to register as self-employed

In some cases, expat freelancers can live in a country without registering there as self-employed. This is essentially the ‘digital nomad’ route. Here, freelancers remain registered with their country of origin for tax purposes. If you’re staying in the country temporarily, this can offer a smoother transition. 

But in practice, if you’re moving permanently, you’ll almost certainly end up registering in your country of residence sooner or later. In many European countries, this is the only way to access public healthcare entitlements. You should check what self-employed health insurance options are available. If necessary, consult a tax expert or lawyer to better understand the implications for your situation.

Purchasing international family health insurance is similar to getting coverage for just yourself. However, there are some crucial differences to be aware of – which we’ll unpack in more detail through the rest of this article. 

When you’re buying your international family insurance policy, you’ll generally have a choice of different coverage types. The most basic plans cover emergencies and acute in-hospital treatment. More expensive plans might offer palliative, rehabilitative, or preventive care as well. 

Therefore, it’s important to carefully consider what coverage you need before signing the contract. 

## What is international health insurance vs. travel health insurance for families? 

International health insurance and travel insurance are often mixed up – but they aren’t the same thing. Travel insurance is designed for short trips (i.e. vacations, work projects). International health insurance, on the other hand, is for expats living abroad. 

The difference is important. Travel health insurance doesn’t generally offer the coverage that expats need. Most only cover very basic healthcare requirements, like emergencies and acute care. They also routinely offer non-health-related coverage, e.g. for flight cancellations. This is useful for a vacation – but not much use if you live in the country you’re being insured in. 

Instead, international medical insurance policies range from essentials-only coverage to more extensive policies. Here is an example of some of the main differences between travel and international health insurance:

## What does international health insurance for families cover?

When it comes to international health insurance, there are several different options available. Most providers will offer around three or four different ‘tiers’ of coverage. These can range from essentials-only to widespread coverage. 

But here’s the challenge: Not all policies will be the same between different providers. A ‘basic’ package might look very different from one provider to the next. So how do you know which plan you need? 

To answer this question, it’s helpful to explain what options you might find in different plans – so you know what to look out for. Here are some of the main options: 

### Emergency care

As the name suggests, this covers the most urgent care. Here, we’re talking about unplanned treatment like A&E visits, ambulances, and acute hospitalizations. This should be covered in all international or expat health insurance plans. 

### Outpatient/GP visits

This includes consultations, diagnostic tests, and prescriptions. It is usually absent from basic tiers, included with annual sub-limits at mid tier, or available with no/higher limits in premium plans.

### Routine/preventive care 

Includes proactive care, checkups, health screenings, vaccinations, and more. These are common in mid-to-premium tier policies. 

### Hospital room

There are also different options for the standard of hospital accommodation, if needed. This can range from shared wards, semi-private, or private rooms – depending on the plan and provider.

### Dental and vision

These are commonly offered as an optional add-on, regardless of the tier. However, more premium plans may offer it as standard. It covers routine dental treatment like fillings, checkups, or orthodontics. It also includes eye tests or corrective eyewear.

### Maternity/pregnancy

Coverage for pregnancy-related treatments is generally only available in the most premium policies. This can include prenatal consultations, delivery, and postnatal care. Generally, you cannot claim for maternity-related treatments until a waiting period has passed – commonly 10 months to two years. 

### Chronic/pre-existing conditions

This describes any ongoing conditions, particularly those that began before you took out the policy. Coverage for these can range widely between different providers. Sometimes, insurers will offer coverage, but only after a waiting period. Otherwise, coverage might only be available in the most premium policies.

## What are the common coverage gaps for family health insurance? 

When you take out an international family health insurance policy abroad, it can be difficult to understand exactly what you are and aren’t covered for. As a result, expat families can be easily caught out if they haven’t got the coverage they think they do. Commonly, we see the following gaps causing the most issues: 

* **Routine care:** This is rarely included on basic plans. In mid-tier plans, you might find that children are covered for vaccines and routine checkups. However, coverage for more ad hoc requirements is less common.   
* **Maternity/pregnancy:** This is commonly offered as an add-on. If you include it, it will likely still be subject to a waiting period – which can be as much as two years. If you’re already pregnant, you’re unlikely to be covered for maternity-related expenses.   
* **Mental health:** Mental health coverage is sometimes offered in mid or premium-tier plans, though often subject to a price cap.   
* **Chronic/Pre-existing conditions:** Conditions like asthma, ADHD, or eczema in children could be treated as pre-existing conditions. In this case, they may be excluded or deferred under moratorium rules. This is even the case if they were minor or well-controlled.

In all cases, it’s important to consider these details thoroughly before choosing your plan. If necessary, get in touch with the insurer to clarify what is covered and what isn’t. 

## How does international medical insurance for families work?

In most cases, family health insurance packages work in very similar ways to standard policies. The key difference, of course, is that the policy will apply to multiple people. This creates some important differences that it’s important to discuss here:

### How do age limits for international family health insurance plans work? 

Family insurance plans generally include age limits. These work slightly differently for adults and dependents:

* **Adults:** Insurers will generally have a maximum age limit: 74 is a common example. This is because older people tend to have higher healthcare requirements.  
* **Dependents:** Newborn babies can typically be added soon after birth (see next section). But most policies will have an age limit on how long somebody can be considered a dependent. 18 is the most common, but some plans extend this to 21 or 25 in certain circumstances.

### How are family expat health insurance costs decided?

As a general rule, you can expect your family health insurance premium to grow as you add more people to the plan. Some will work on a ‘price-per-head’ basis, meaning the costs grow gradually with each additional member. Others may offer a flat cost, perhaps for three or four people.

Some plans will also add discounts for additional family members (particularly dependents). You may find that your per-head cost reduces as the family grows larger. 

### How to add or remove people from an international family health insurance plan

Family insurance allows you to add or remove adults or dependents, but generally only in certain circumstances:

* **Newborns:** To add a newborn child, you will have to register them on the plan shortly after birth. Your policy will usually have a defined process for this. For instance, many require you to add newborns between 30-60 days after birth.   
* **Dependents:** Other family members can generally be added or removed when the policy renews. Some may allow you to do so on an ad hoc basis. 

In all cases, the specific details should be outlined clearly in your policy terms and conditions. 

## How to choose the right international family health insurance plan

When it comes to family insurance, there are plenty of options available. This can make it challenging to understand which option is the best for you. With different levels of coverage, premiums, usage caps, and prices – there’s a lot to compare. 

Here are some tips to help you evaluate the options on the table and choose the right policy for your family:

### Check the visa requirements

Many visas require health insurance as a precondition. In these cases, the policy must usually fulfill several [visa-specific requirements](https://feather-insurance.com/en-ww/blog/international-health-insurance-visa-seekers). These can include details like ‘minimum €30,000 coverage’ or ‘no co-pays/deductibles’. You might also need to purchase from an insurer that’s registered in the country you’re moving to. If you need a visa, it’s vital to check these details first. 

### Understand your coverage needs

Before comparing any policies, take stock of what your family actually needs. This could include details like how often you visit the doctor, or if a teenager might require orthodontic treatment. 

You should also consider whether you’re planning on having more children in future. Be aware that young children, teenagers, and pregnancy all have very different healthcare requirements. 

### Consider pre-existing conditions

When you declare a pre-existing condition, insurers will typically offer one of three outcomes:

* **Excluded permanently**: The condition is written out of your policy entirely, meaning no claims related to it will be paid. This keeps your premium low. It is a reasonable choice if the condition is minor, well-controlled, or unlikely to generate significant claims.  
* **Covered after a waiting period:** The condition is excluded for a set period, typically 24 months. After this, it will be covered as long as you didn’t need treatment during the waiting/moratorium period. This is the most common middle-ground option and suits families where a condition is currently stable. In this case, you accept short-term risk in exchange for long-term coverage without permanently high premiums.   
* **Covered immediately, with a premium loading:** The insurer covers the condition from day one, but charges a higher premium to reflect the additional risk. This is worth paying for conditions that are active, recurring, or expensive to manage. 

If you’ve got a pre-existing condition, it’s helpful to get quotes for all three options. Then, weigh the premium difference against how likely you are to actually claim for that condition in the near term.

### Check deductibles and co-pays

A deductible is the amount you pay out-of-pocket before your insurer starts covering costs. A co-pay is a fixed contribution you make each time you use a service. 

These generally result in lower premiums, so they can work for some. But it’s a careful balancing act. If you find yourself needing a particular type of treatment a lot (e.g. GP visits), these co-pays can quickly add up. 

When choosing your policy, therefore, it’s important to consider both the monthly premium *and* the risk of out-of-pocket expenses. Again, make sure to check your visa requirements here, as many do not allow co-pays or deductibles of any kind.

What if you're seriously ill somewhere without adequate medical facilities? Evacuation cover pays to transport you to the nearest hospital. It's one of the biggest financial risks of travelling uninsured.

### Routine and outpatient care

This is where plans have the biggest differences. Budget and emergency-only plans exclude GP visits and outpatient appointments entirely. International health plans typically include them.

### Mental health support

Mental health coverage is increasingly included in mid-range and premium plans. Always check whether it's included in the specific tier you're considering. Also see if there's a session cap or annual spend limit.

### Pre-existing conditions

Most digital nomad health insurance plans exclude pre-existing conditions. That's any illness, injury, or chronic condition you had before your policy started.

AXA is a premium option for long-term expats with modular, customisable plans. Premiums are typically higher than nomad-focused providers. Excluding the USA from your area of cover brings the price down significantly. Emergency USA coverage is still included on most plans.

## Do digital nomads need health insurance for visas?

In most cases, yes. Private health insurance is a requirement for most digital nomad visa programmes. Requirements are set independently by each country's immigration authorities. So, minimum coverage amounts, accepted insurers, and documentation standards all vary. Even within the same region, no two programmes are identical.

### Visa insurance requirements

The Schengen Area requires a minimum of €30,000 in medical coverage under [Schengen visa rules](https://home-affairs.ec.europa.eu/policies/schengen-borders-and-visa/visa-policy_en). Greece, Portugal, Germany, and several others have the same or higher requirements. Requirements vary by country and visa type. So, always check the specific programme you are applying for.

### Tax residency

Your tax residency affects plan eligibility and how claims are processed. Most countries use a 183-day threshold. Spend more than that in one country and you typically become a tax resident there. A visa doesn't equal tax exemption.

### How your insurance needs change with visa status

#### Spain

Spain's Digital Nomad Visa requires private health insurance authorized to operate in Spain. The policy must have no co-payments or deductibles. Once you're registered with Spain's social security system, public healthcare becomes available. Private cover may no longer be needed.

#### Portugal

Portugal's D8 visa requires private health insurance at application. The Portuguese SNS is the national health service. Residents who register with it can eventually use the public system. You need qualifying private cover to get through the door, though. 

#### Thailand

Moving abroad in retirement changes everything about how you manage your healthcare. It’s a new world, and one with new challenges when it comes to age-related needs. 

More than 90% of adults over 65 have at least one chronic condition. That’s [according to the Centers for Disease Control and Prevention (CDC)](https://www.cdc.gov/chronic-disease/about/index.html). Most standard expat policies are designed to exclude or restrict these, and there are also other hurdles to overcome. It means finding the right policy takes more thought than most people expect.

[Expat health insurance](https://feather-insurance.com/en-ww/blog/international-health-insurance-guide) is very different from what most retirees are used to. And although many retirees think standard cover from back home will be enough overseas, it won’t. There’s a lot to figure out.

## Why is standard expat health insurance often not enough for seniors?

Many retirees assume any international policy will do. In practice, a few things make this harder after 60 or 65.

### Age limits

Some international insurers set maximum entry ages, often 60, 65, or 70. If you’re past that threshold when you apply, you may be turned away. Always check the entry age before comparing providers.

### Pre-existing conditions

The older we are, the more medical history we carry. Many standard expat policies exclude pre-existing conditions outright or apply waiting periods. Are you a retiree managing a condition like diabetes, heart disease, or hypertension? This can leave a serious gap.

### Rising premiums with age

Premiums increase with age, sometimes sharply. Look for lifetime renewability guarantees. Make sure to ask whether your rate is locked at the age you join.

### Cross-border medical needs

Standard expat policies are often tied to a specific country or region. Will your healthcare needs cross borders? Or will you want specialist treatment elsewhere, such as your country of origin? If so, that limitation can leave real gaps.

## Can you use Medicare overseas?

It’s the most important question for US retirees. The answer is mostly no.

Original Medicare (Parts A and B) doesn’t cover healthcare outside the United States. We confirmed via [Medicare.gov](https://www.medicare.gov/). There are only very narrow exceptions. One example: a foreign hospital closer than any US hospital during an emergency on US soil.

Living in Spain, Portugal, Mexico, or anywhere else outside the US? Best to plan as if Medicare provides no coverage at all.

Medicare Advantage plans (Part C) also typically don’t cover care abroad. Some include limited emergency travel coverage, but terms vary widely.

### What about Medicare Part B?

Medicare Part B [charges $202.90/month in 2026](https://www.cms.gov/newsroom/fact-sheets/2026-medicare-parts-b-premiums-deductibles). It can impose a permanent late-enrollment penalty. If you plan to return to the US eventually? It may be worth maintaining Part B. Check with a Medicare advisor before you decide.

We've established that Medicare provides little to no coverage abroad. So, most retirees use international private health insurance. It's their direct replacement while living overseas.

## What should seniors prioritize in a plan?

Pay close attention to these factors:

* **Coverage limits:** Aim for a high annual or lifetime maximum. Especially if you plan to return to the US for treatment.
* **Pre-existing condition terms:** Are pre-existing conditions excluded? Or covered after a waiting period? Or covered from day one? Ask this directly. The answer differs considerably between providers.
* **Emergency evacuation and repatriation (transport home if seriously ill):** Will you ever need medical evacuation from abroad? It can cost $25,000 – $250,000+. That’s according to [the CDC’s Yellow Book 2024](https://www.cdc.gov/yellowbook/2024/travel-by-air-land-sea/travel-health-insurance). Make sure it’s included in your policy, not just an add-on.
* **Prescription coverage:** Are prescriptions covered? Does coverage extend to medications purchased abroad? Be sure to check.
* **Lifetime renewability:** Insist on guaranteed renewability. You don’t want to be dropped mid-retirement because you’ve made a few claims.

### International private health insurance

International private health insurance provides healthcare for expats living outside their home country. It’s sometimes called global health insurance.

They typically cover hospital stays, outpatient appointments, specialist visits, and diagnostics. Prescriptions are often included too. 

### Local insurance in your destination country

Buying from a local provider can be cheaper. It also integrates well with the local healthcare system. But there are trade-offs. Coverage is limited to one country. Documentation is often in the local language. Eligibility rules vary. Worth exploring if you plan to stay permanently in one place.

### Travel insurance: not enough long-term

Travel insurance covers emergency treatment only. So, no routine care, no chronic condition management, no prescriptions. If you’re living abroad, it’s not a substitute for proper health coverage. For shorter trips, travel insurance is worth a look.

The right choice depends on a few things. Those are your condition, destination, and how much certainty you need.

A 65-year-old couple needs $388,000 in savings just to cover healthcare in US retirement. That's according to [Milliman’s 2025 Retiree Health Cost Index](https://www.milliman.com/en/insight/retiree-health-cost-index-2025). So, healthcare in retirement can be expensive wherever you live. The right plan makes it manageable.

### What drives your cost

* **Age:** The single biggest variable. Expect premiums to increase meaningfully at each renewal as you age.
* **Coverage level:** A basic plan covering emergency and hospital care costs less than a full plan. Add outpatient visits, mental health support, and prescriptions and the price rises significantly.
* **Destination and area of coverage:** Plans including the US cost substantially more. As the Feather pricing above shows, adding US coverage can multiply your premium by three or more. Exclude the US from your coverage area if you don’t plan to seek treatment there.
* **Pre-existing conditions:** Your medical history may push premiums higher. It depends on how a provider assesses your health (called underwriting).

## A practical example: choosing expat health insurance with a pre-existing condition

### The situation

Maria is a hypothetical 67-year-old retiring to Lisbon, Portugal. She has well-managed hypertension and takes daily medication. Maria doesn’t plan to return to the US for treatment. She needs a plan that will actually cover her condition.

### The options

She looks at three options. A **basic international plan** covers inpatient and emergency care only. But it typically excludes outpatient care and prescriptions. Those are the two things she needs most.

A **comprehensive international plan** costs more. But it covers specialist visits and ongoing prescriptions. It also gives her English-language support from day one.

A **local Portuguese private plan** is even cheaper. But it only covers Portugal and paperwork is entirely in Portuguese. For Maria, the comprehensive plan is the right fit. The basic plan looks affordable but leaves her most important needs uncovered.

### The question that matters

With hypertension, Maria knows her condition will be scrutinized. Many plans will exclude it outright or impose a 3 to 12 month waiting period. So, before comparing prices, she asks every provider one question. “Will you cover ongoing hypertension medication and related specialist visits from day one?” Most can’t. The ones that can become her shortlist.

## How pre-existing conditions are handled

This is where most retirees get caught out. Here’s why:

### Medical underwriting

Most providers ask you to complete a health questionnaire when you apply. This is called medical underwriting. The insurer uses your answers to decide whether to cover you, at what price, and under what exclusions.

### Waiting periods

Some providers cover pre-existing conditions after a waiting period. That’s typically 3 to 12 months. During that time, conditions you had before taking out the policy won’t be covered.

### Guaranteed-issue plans

A small number of providers offer guaranteed-issue plans. These accept all applicants without a health questionnaire. They cover pre-existing conditions from the start. Such plans tend to be more expensive and relatively rare. Most plans are underwritten.

We asked Julian Hennig, Head of Insurance at Feather, for his thoughts. “For retirees over 65, the biggest mistake is focusing on price. Coverage stability is even more important. A cheap plan that excludes your most important conditions isn’t really insurance. It’s a false sense of security.”

## Which international health insurance plan is right for you?

Here’s what to do depending on your situation:

* **You’re 65+ with Medicare:** Get a comprehensive international private health insurance plan before you leave. Medicare won’t cover you.
* **You’ve got pre-existing conditions:** Don’t buy on price. A cheap plan that excludes your conditions offers no real protection. Look for guaranteed-issue plans or those with waiting periods under six months. Ask every provider directly: will my specific conditions be covered, and from when?
* **You’re retiring to a country with strong public healthcare (e.g., Portugal, Spain, France):** A lower-cost private policy covering the gaps in the local public system can work well. Avoid this route if you have complex pre-existing conditions. Or if you want guaranteed English-language support.
* **You plan to travel frequently or [split time between countries](https://feather-insurance.com/en-ww/blog/international-health-insurance-digital-nomad):** A worldwide policy is the best choice here. Avoid regional plans. They look cheaper but leave you uninsured the moment you cross into an uncovered country.
* **You’re on a [retirement visa](https://feather-insurance.com/en-ww/blog/international-health-insurance-visa-seekers) (e.g., Spain Non-Lucrative Visa, Portugal D7):** Check the visa requirements before you shop. Not after. Some specify minimum coverage amounts or require repatriation cover.

## How do you compare providers?

Beyond the monthly premium, check these:

* **Deductibles and limits:** A lower deductible means less out-of-pocket cost when you claim. But it means higher premiums. Annual, lifetime, and per-condition limits vary too. Read the policy schedule carefully.
* **Direct billing:** Does the insurer pay hospitals directly? If not, you pay upfront and claim back. This matters most in a medical emergency.
* **Exclusions:** Watch for pre-existing condition exclusions. Also look out for country-specific restrictions. Remember conditions that arose before the policy start date.
* **Claims process:** Can you file online? How quickly do they pay? Check independent reviews before deciding.
